This form can be found in the pool room in the Tuscany Clubhouse.  Please place sealed envelope with waiver and check (or cash) in top drawer of billiards room cabinet near table 1 and drop a quick text message to the Club Treasurer Harold Erker at (720) 560-6682 to let him know it's there, available for prompt pickup.
